Home and Community Care (HACC) Services
Referrals for eligibility are directed and determined through or by Tasmania Carepoint (1300 769 699).
Community Aged Care Package (CACP) and Extended Aged Care at Home Package (EACH)
Clients requiring a CACP or an EACH package must be assessed and approved by the Aged Care Assessment
Service (62 227 274). The clients name will then be placed on a waiting list for when a vacant package becomes available from an approved service provider.
Nursing - Veterans are required to contact a GP for a referral for nursing services prior to service being provided by SECC.
Veterans Home Care - Veterans are required to contact Care Assessment Consultants(62 447 770) for an assessment prior to services being provided by SECC.
Referrals can be received from:
• Allied Health Professional
• General Practitioner
• Relative or family friend
• Self referral
